  • Develop personalized management plans for patients with type 2 diabetes (T2D) based on current guidelines, the most recent evidence, patients’ comorbidities, and shared decision-making with an educated patient

  • Assess glycemic and extra-glycemic control on a regular basis including evaluating patient adherence to their current regimen and medication efficacy

  • Intensify antihyperglycemic therapy as needed incorporating new data, new treatments, and guideline modifications.
